help spousal sponsorship!!!

Hi everyone,
I need help regarding my sponsorship for my spouse. I quited my job in november to go back home in the philippines and we got married january 2015. I stayed there till april and now im unemployed. And I am a canadian citizens.

My questions are:
1) Do I need to change my last name first before I can apply to sponsor him? If I don't change it, will there be an issue?

2) How do I begin with the process? Where do I get the application to send to mississauga?

3) How do I pay for the application? I believe it's $1040?

4) Does my husband has to get his medical exam from st. Lukes before i pass the application to mississauga?

I read few forums here but I'm still confused. If anyone knows the answer to my questions please reply. Thank you.

1) No, you don't need to change and will not be an issue.

4) It's recomended to avoid any unnecessary delays, to submit everything upfront, including medical and payment fees.

Thank you for your reply. I guess i have to tell my husbandto get his police clearance and medical exam first before I send the application.
