I am applying for GCMS notes for myself. I have a friend who is Permanent Resident of Canada, so I will pose as that friend while filling out my own form. Now, I am confused about consent form as what to write in following sections:
1. Section A: You must clearly write your name and date of birth. (Should I fill out my details or my friend's details here?)
2. Section B: That is not applicable in my case
3. Section C: You must write your designated individual's contact information. (Should I fill out my details or my friend's details here?)
Please reply ASAP as I have to apply for GCMS notes today. :-X
