confusion about consent form IMM5744E

hello,

I received gcms notes, but some sections which contain my spouse's information are blank. So, wanted to apply once again with consent form IMM5744E. But I am confused while filling some info in the form..

1. Whose information would be there in section A,B, and C in IMM5744E ?
2. While applying GCMS, *Are you requesting information on your own behalf?; what would be the answer?

Heyyyyyaaa MapleLeafCan,

I just got my CAIPS notes and I had the same confusions while applying, there were no answers here so I decided to apply anyways and find out for myself and it turned out successful

So, I'll share my understanding on how to fill it.

Section A - Is the information of the person's information you require from CIC.

Section B - If the person also requires information or updates regarding their spouse's application from the notes. Should fill the spouse's information there.

Section C - Is the information of the Canadian resident applying for the notes on your behalf. Signature of representative has to be his signature.

Section D - Is required to be filled by the parent of the person if the person is below 18 yrs old.
