Help Please: CAIPS Notes

I would like to order for CAIPS notes to know my PR application status.

Do I need to still fill the consent form (Imm5475) or is it enough if I fill IMM5563 along with the 5$ fee.

Please let me know if I need to send any additional documents like passport copy, wp copy etc.

I would say wait for another few days before ordering CAIPS, as it will take 6 weeks to reach them. Else you can send a case specific inquiry to CIC via email regarding status of your application. FOr caips you will need a representative who is authorized to order them on your behalf and of course representative form will have to be filled in.
