My WES report states ( As per my degree)
Last Name, First Name Daddy Name Mother name (FORMAT)

My passport : Last Name, First Name

Now I have done WES and received ITA. what should I do :
1. Decline ITA, re do the WES and then reapply
2. Notary/Affidavit explaining that my WES report has my father name and mother name included.

Has some one faced the same issue? If yes then please also let me know your course of action.

Please help guys.

You can use a same name affidavit.
 
You can use a same name affidavit.
you can also write an email to WES to update your name on the ECA certificate. I did the same thing and sent the photocopy of the passport as a proof. WES accepted it and re-issued a new certificate to me with the new name. Just write an email to them explaining ur situation and see there response. Generally they will reply in a couple of days.

Your surname is your family name. It is also called your “last name.” When filling out applications, type your surname as it appears on your passport, travel or identity document.

Do not use initials.

If you do not have a family name on your passport, travel or identity document, enter all your given name(s) in the surname field and leave the given name field blank. Do not enter “Not applicable,” “*” or “NA.”
