Help a live-in caregiver to fill out the application form for the OWP!!!

Hi, I am asking for help as I am getting ready to apply for my PR in a couple of days. Everything is clear about that application, but lots of questions about the OWP application form.

1. There is no a special form for the "open work permit". Do I use the same as "to change conditions...", but write on the top APPLYING FOR OPEN WORK PERMIT?

2. If so, this form asks me for the LMO number, contract, a letter from the employer and some other details that I knew what to write when I was applying for my present work permit extension, but what contract and LMO should I provide if I don't have a new one, I want to be free to go and look for a new job?

Please, help, somebody!

Use the change of conditions form at http://www.cic.gc.ca/english/pdf/kits/forms/IMM1249E.PDF
In question 10, checkmark that you want to change conditions and write in the space: "Applying for an open work permit to be granted at first stage approval of my PR application as a live-in caregiver".

You do not need to put anything on how long you want to extend your stay in Canada. They know you are applying for an open work permit while you are waiting for your PR so I am sure they will give it to you for long enough to last until your PR comes through.

I do not understand the question. Suppose it's about the client ID number or work permit number? If you look at the instructions at http://www.cic.gc.ca/english/information/applications/extend-worker.asp it does not say if they consider your work permit number to be your client ID number. They just say if you know the client ID, then write it down and if you don't know it, leave that field empty so I would say the safe bet is to leave it empty.

The client ID is actually the eight digit number on your work permit. It has four numbers, a dash (-), and four more numbers. (E.g: 2222-3333). Since you know it, you can as well fill it in the space provided on the change of conditions application.

Yes, I have always included the client ID whenever changing conditions or extending my stay in Canada. But it's not a critical thing. Nothing negative will happen if you don't put it. They will still be able to pull up your file using your name/DOB.. but it's much easier for them with a client ID.
