Point here is, they could ask for RPRF anytime, but usually near the end. The suggestion was to get the gcms notes to see the true motve for asking for the refundable fee.

I got you, but I guess this is near the end. They (8 months ago) requested additional documents as proof for genuine marriage and said they will conduct a final review. After documents were sent, now they requested for a re-medical and to pay the fees. So I guess this the last step before the passport request?
 
It would seem like it, but one can never be sure with CIC. Still, to specifically ask for a remedical and the RPRF does sound like they will issue the visa.
 
Medical exam completed and submitted. Waiting to see how things unfold. Just wondering if I should send a copy of the emedical sheet to the VO office?

Yes you should send a copy of the eMedical paper to VO.
