Please don't suggest people click/navigate to unknown links, that is a security risk for anyone.

If anything the full url should be visible, don't use url shorteners.

Please describe your problem as much as you can.

The IMM 5257 can only be uploaded, if it is in the original form. When it is complete, it can be validated, and saved as a pdf. Then, it can be uploaded. If you rescan it, it will not upload.
I also had problems with some forms, when using Chrome browser. I switched to IE, and all was fine.
