Haven't received my PR card yet

Hi Members,

I landed in Canada in December 2018. At Passport control, office told me that I can give my US address, and my Green Card will be mailed to my US address. Other reason was that I didnt have any Canadian address to give.

It has been more than 6 months and I haven't received my PR card. Now, I have a known Canadian address where I can ask them to mail me the PR card.

So, what should i do now ?

1. My search shows that I should fill this form :
https://www.canada.ca/content/dam/ircc/migration/ircc/english/pdf/kits/forms/imm5451e.pdf
and fax this.
Is that correct, given that its past 6 months I haven't received card, and they most probably never mailed me ? If not, suggestions ?

2. If Point 1 is the way to go, it asks for providing the "Client Id" and "PR card No. (if applicable)".
I only have COPR at this point of time, which has following numbers which I can see :
- UCI,
- App No and
- Document No.

- I believe with just COPR in hand, I don't have PR card no. with me. Please correct if wrong.
- And, where do I get my "Client Id" from ?

First go to your MyCIC account and try to link your PR card application. Most likely, you won't find it because they don't mail PR cards outside Canada. The CBSA officer must have been misinformed.
Then you can file a replacement card application, along with the above declaration. You may need to pay a fee. In the declaration, fill whatever you can.

As u suspected, I wasnt able to connect while trying to connect the applications.

By the way, ClientId and UCI are same (Ref : http://www.cic.gc.ca/english/helpcentre/answer.asp?qnum=012&top=4)

Quick question :

- Given that I will be FAXing the filled form, where would I pay the fees for sending new card ?
