Ok, so after very careful and thorough review of the notes, I found that it says that IMM5490 was missing! It most definitely WAS NOT! How could my papers have been approved and transferred from CIC if this form is missing! Its so important!! UGGHHHH. what does this mean? what do I do?? SO frustrating!! If you guys knew how many times i went over and checked that EVERYTHING was there! and on top of it, thats the cute paper that gives allll the info about our relationship! Im so confused!

i googled right away that form number i am sure you did not miss this one as it is quite important....did you make copies of your application? if so scan them that form and send it via email to the embassy...cannot hurt

it is probably not missing..they just either put it out of order or Mississauga lost it but pretty sure they have found it or they would have contacted you by now.......having said that.....what don't you email them and advise that you have the notes and that you are attaching a copy of the form that was sent in the original application.....please tell me you kept a full copy of your application!

I am 100000% sure it was not missing, I specifically remember that one from the beginning. DO you think we should refill it out and send it again? :'(

right now before doing that I would email them and confirm that it is in fact missing....the person who looked through your file might have missed it......like you said Miss. would have returned or asked for it earlier....but email and offer to resubmit if needed....but don't fill one out right now.....use both their email addresses...seems to work the best....explain that you ordered your notes to see where your file was at and in them is says this form is missing as them to confirm that and see if they need a new one....

i had put everything the box to send and i looked at it and said WHAT if they lsoe this box? So took everything out and made copies!!!!! golden rule for sure

I know!! I swear Im not a stupid person, I was just so overwhelmed with papers at the time, and had so many i didnt think to keep a double of everything. just wanted it OUT OF THE HOUSE! lol But I have my copies on computer, and he has his, so were good. we got it all!
