GMCS still not clear

Hi again
Please I'm still confuse. Who has to sign section C for the release of information?
I fill in section A and sign it. My hubby section C I was told but then something about representative was mentioned and I thought I had to sign it albeit the fact that my husband has to fill in that section. Where do we send the form to? Is there a website or an address to submit the GMCS form and lastestly where do we pay the $5. Is the form to be sent online after my husband scans it or by mail? Thank you guys so much for all your help. Cheers
Ps I filled in section A well as B and according to my hubby it is wrong. That part is for me to fill and sign and what part is for my hubby to do? Please help

Section A filled out by principal applicant
Section B left blank
Section C by sponsor - Sponsor is in Canada and requesting the notes.
Also the sponsor will need to sign on the representative line
