Please I'm still confuse. Who has to sign section C for the release of information?
I fill in section A and sign it. My hubby section C I was told but then something about representative was mentioned and I thought I had to sign it albeit the fact that my husband has to fill in that section. Where do we send the form to? Is there a website or an address to submit the GMCS form and lastestly where do we pay the $5. Is the form to be sent online after my husband scans it or by mail?
