I've got a job offer from one of the employers from Alberta under GSS. Apparently, they already had an LMIA approved and now they have submitted my details to the government to include it in the LMIA.
Meanwhile, I'm collecting documents required for work permit applications for me and my wife.
Can anybody please help me with the following questions?

Can anybody please help me with the following questions?
- Are medical examinations and police clearance required for both me and my wife?
- In the work permit application (IMM 1295) there is a question asking for 10 years of employment history but only 3 boxes to fill in the details. I have more than 3 previous employers, so, how do I go about this? Should we mention only the last 3 employers?
- In the online work permit application. How long are you planning to stay in Canada? What should be the option? "Temporarily - more than 6 months" or "Permanent"
- I am applying a work permit for my wife as well. So, under family information, there is a question "
Why is your family member visiting Canada?" What should be the type of Visa for her?
