GIC certificate question

Hi, when we receive an email for confirmation of a GIC approved certificate with 3 attachments including "Scotia® Investment Account Investment Directions" form.

Do we need to sign that form before we submit with our file?
If yes do we need to complete the entire form??

It’s ok even if you don’t submit that form..
Just submit the certificate
