Anyone know if they ask you to resend your consent form again, do you go back to the bottom of the pile again and have to wait 31 days again until they send your notes? They gave me until February 9 to get the new consent form in.

Happened with me. It will not go to the bottom of pile but a delay of week can be expected. 5 days in my case.

same here. no worry, the notes will arrive soon. i dont want to email nagging them every seconds just to piss them off even more. like the saying goes "dont piss off the person handling your food"

I had to wait another 15 days

I got my gcms notes its written under group name heading that group 1 and created by and thing i want to knw that group name secondary review and status is open and under notes that paid representative is in good status

Hi all, I saw this question (unanswered) on another thread, and maybe someone here can speak to it. The poster asked what is meant by "associated with" an organization in the background information form. It concerns me because I was in school decades ago, and I doubt that I remember every activity or club in which I was involved. I may be overthinking this, but would omission of something like that cause problems? I don't want to be accused of misrepresentation because of either forgetting or misunderstanding how they define "association." I limited my answers to what I'm sure I've been a member of or donated money to.
