Good day guys,

Advise naman po,

I was required by CEM to submit my bank book, a signed narrative explaining my POF, additional Personal History and Address details and also a new NBI Clearance.
I have completed all required documents except for the NBI Clearance - I went to NBI to get a new one earlier this day and to my surprise the clearance will be released on June 11th since, for the first time, my record had a "hit" on it.

Is it advisable to send the completed documents to CEM now and then I would just send my new NBI Clearance on a later date?

Quick question lang po ka-forumates hehe yung sa MR, we only received 2 forms, 1 for me and 1 for my hubby.
Wala kaming napansin na add'l documents na ipapasa, ganon din ba kayo?
May nababasa kase ako dito na nagsu-submit (ulit) ng NBI, police clearance etc. Just making sure po.

In some cases, like mine, MR lang ang nirerequire. If kailangan ng additional docs, nakaspecify yun sa cover letter ng CEM. I got the following from CEM:

"Complete medical examinations for yourself and, if applicable, for your spouse/common-law
partner and all your dependent children, whether they are accompanying to Canada or not.

YOUR MEDICAL FORM(S) AND INSTRUCTIONS WILL BE SENT SEPARATELY TO
THE SAME E-MAIL ADDRESS(ES) WITHIN 72 HOURS.
You should make arrangements to undergo the required medical examinations within thirty
(30) days from the date of this letter.

 Principal Applicant:
o RPRF: Pay the Right of Permanent Residence fee

 Dependents:
o RPRF: Pay the Right of Permanent Residence fee"

Pag kailangan ng PCC or other docs, naka-indicate as bullets. Hope this helps.

A ganon ba, nag - worry lang ako ng slight hihihi... Salamat lloyrine..Hindi pa expired yung NBI, i think 1 year ang validity non so pasok pa.

I think it will be the best if you can send everything in one go. Incomplete documentaion won't hasten your application anyway

hi, ganyan din ang gusto kong gawin dati kasi yung akin 6yrs ago na. Pero hindi din kasi makakapag start ng afb and culture kung walang MR kasi yun yung hahanapin. Kung magpapatest ka sa iba ngayon, dodoble lang bayad mo kasi mahal din ang culture. Pero kung gusto mo makasiguro na negative ka nga talaga, best siguro na magpatest ka sa accredited hospitals din, (kung nasa ph ka, sa st. Lukes). If ever, pwede mong ipasa yung result as reference pero papaulitin parin nila yun sayo kasi gusto nila makasiguro na sayo nanggaling mismo yung sputum. Kung may records ka pa ng treatment mo, like xray films makakatulong yun ng malaki. Sakin, hindi na ako pinaulit after lumabas ang result ng culture. Ihanda mo nalang yung medical certificate mo na may nakalagay na treatment period, medicines taken at diagnosis.

*if ever pala nagpositive yung afb or culture, it means 6months delay kasi magtretreat muna dito sa pinas

Hi po forummates and JayJas2007. Gusto ko po itanong sa case na may PTB history 9-10 years pa.. pede na ba mag pa sputum and AFB test kahit wala pang MR. Kung sakali unahin na namin?
Saan mo kaya pedeng mas maganda magpa test?

Hindi na po kailangan yung online na application. Pumunta ka lang dun, mga 6am para ikaw ang una sa pila. Hehehe. Para sure tawag ka na din.

anyone here po na nagpa-medical sa St. Lukes (BGC)? I'm trying to fill-up required information sa online application.
