As much as possible please No. Gawa ka separate sheet with all your main duties and lagay mo sa reference form na "Please see attached extra sheet for the answers". Ganyan din ginawa ko sa personal history, addresses and education history. Also don't forget din na fill-out-an yung nasa reference form baka makalimutan.

As much as possible para lang sure... dun sa separate sheet, lagyan niyo rin ng Signatory box and date box sa ilalim kasi remember, extenstion yung paper na yun nung CIC doc and means na pinanunumpaan na true yung nakasulat.

mapapansin nyo pag filled out thru a computer, some of the fields changed color if they are not applicable after answering related fields.
It means it is not applicable. Yet, you have the option to write not applicable after printing.
What I am trying to say is, when it is gray in color, the pdf system didnt allow you to fill it out coz it is not applicable.

Share ko lang.
Eto naman ang turo ng consultant for the personal history and additional family information. Pag di kumasya, use the same page of the form, write those not included in the main form. Kasi before, i attached an extra sheet. Pero sabi what they( consultant) will do is to print it in the same page of the form, don lang sa portion na yon. Other fields blank.
No need to attach extra sheet.


Sa palagay ko naman sa economic form, if you attach another document listing all your duties and responsibilities, make sure you put the title of the form, your name and sign it. baka madetach lagot!

Halos same lang, depende lang talaga minsan sa oras ng pagdating sa sorting facilities on each stop, kung na late ng saglit, minsan naiiwan for the next day for shipping. Although ako I used DHL for Nova Scotia and then UPS for WES. I read kasi somewhere that mas malapit ang DHL sa NS office. (4 working days)

Yung sa generic app form po kasi nakagray-out din yung from and to dates sa question #10 (Current country of residence pag pinili mo yung status na 'Citizen'. Ano po ginawa nyo?

When it is gray i left it blank.

I cant remember the exact words in the guide. It differentiated when you fill out the forms thru computer and by pen. Related sa validation. If thru computer,fields will
turn red if you need to fill those and you didnt. If you didnt the form will not validate.

If by pen, there is no way to validate it. So here, you have to fill out the field with N/A or not applicable. Also the bar codes thing is not applicable to forms filled out
by pen.

I myself had hard time validating and producing the bar codes. I successfully validated it but still not successfully completed it, because the codes didnt show up. NO matter how i tried, it was blocked. I just put it in the remarks in my covering page that i was not able to generate the codes and i ask their consideration.

I believe yours only. Because it is only a summary. The supporting documents from where you copied the words are the ones that your boss must have signed.

If you don't mind me asking. ano yung prompt message nung ivavalidate mo na yung GenForm? nangyari yan sa akin. then bigla rin pumasok at navalidate with bar codes.

When i entered Philippines and citizen, the other fields went gray. Bluish gray to be exact. NOthing. The other fields actually refer to Others.

I have finally decided to include my middle name in all forms. I guess either way should be fine kasi lahat naman na-grant na ng visa regardless kung nilagay ang middle name or not.haha. Thanks everyone :)

Case closed! haha

Before, I put only my first name.When I sent my application for review, nun nireturn sa akin for signature eh sa first name nakalagay is my first name and middle name(which is my mom's surname when she's still single).

Pag nag fill out ako nang forms, rule of thumb pag sinabi as seen in passport, lagay mo nakalagay sa passport. :)
