I have question regarding visa, since you got 3 line on 11-03-15, what is your visa expiry date? Is it one year from 3rd line?

I believe it's counted 1 year from the date of the medical exam.

Some mistake I made during app (self apply):

* forgot to write my name on the Generic Form but I signed it
* forgot to mentioned US visa refusal on initial app, raised CSE and during MR I sent the updated form with corrected answer and explanation on visa refusal

Expiry is 4-3-16..so its on the day they received the result not the 3rd line? Not sure since 3rd line or 4th line has no date. It is probably once they accepted your medical result, 3rd line is just an indicator.

In short, its your medical date plus few days to receive if all is well.

I am taking next month about 3-4 months break for some family reasons, in this period if my site verification happens what will be the situation then? Which type of document should i prepare for that to show them in my absence? More over the address i use in my application that company branch may be closed with in short time then what? should i inform them earlier? I am confuse because I am yet sure about that branch closing, but there is possibility of that. Please help by providing your ideas.

Yes it is better to raise CSE to inform your company status.
But it will not impact on your file processing if you take break for your family reason.
Usually CHC contact applicants first if there is required to site verification or CHC calls to referees phone / email from that last reference letter other than if there is missing to call to the applicant. But if you inform VO then they will contact with you first for their further verification required.
