what if I go there and come back after a month after getting my PR card.
Then go there after 3 years. Will my PR be extended after 5 yrs ?

If after 5 years you've been present in Canada for at least 730 days, then yes.

You will need to visit local police station where you will have to submit an application to get the issuance of Police Clearance Certificate. You may nominate you blood relation to request on your behalf.

Its called Character/police clearance certificate and you will get it from the police station of your area. If you are not in the country, you can nominate someone to get it for you by giving an affidavit/authority letter to him. Requirement may vary slightly from one police station to another but in all cases you have to fill in an application from that is available from the police station. You will have to submit photograghs, ID card copy attested, passport copy of pages 1,2 and 7 attested, copy from Canadian immigration highlighting the requirement of police certificate (you can print it out from the cic website), affidavit from 2 neighbors confirming that you reside at that address (form for this is also available from the police station).

My ques is slightly different.
I want to understand what happens if one does not complete 730 days in the 5 years ?
eg: I go for 1 months then come back and go after 4 yrs that way I would have stayed for only
30+356 within 5 yrs period of getting PR, in such cases do they extend the PR ?
