you should send the original PCC.
you should send the MR acknowledgement which you got from the dr after your tests.
If you have already paid the RPRF online, sent that acknowledgement. else take a dd for 26700 rupees per adult.
first and last page of the pp is enough.
send them all together with a covering letter and the printout of the MR mail which you received from NDVO

My application was received by CIC on June 9,2014. My CC got charged on Sept 25,2014. Living in US. 3142.

For people who have used their CC in India, Please keep in mind that CC companies charge extra Service charge on top of the 1250 CAD, that would be approximately 3000 INR.
So make sure you keep that amount available and also keep in mind the fluctuating exchange rate
Another point for you guys to remember
Make sure you inform your CC company... I had to call them like 10 times from last couple of days to have them make a note of CIC transaction...
Because the transaction is a huge amount around 70K INR, there are chances it might not go through in the first attempt

- Please send photocopies of your updated/valid passports (only front and back Bio pages) for all .

I have a doubt that we have to send attested copies or normal photocopy without attestation.

Please suggest.

PCC : In original ( keep photo copy in your custody)
MR : You have nothing to send. The designated Physician will take care for it.
RPRF : Yes CAD 490 per adult. No fees for children. You can pay in INR too. Send receipt if they asked for.
Passport: First, Last and remarked pages if any.
You can send all in one package. It is better to send cover letter. Indicate all enclosures.
For mailing you can choose any standard courier like: FeDex, DTDC, DHL or Speed post.

You can send them Attested its not that costly.

Can you please tell us which Visa Office and your PER date ?

Do you have any idea why they are asking for it? Did you not send it with your application?

Thanks for the quick response. I received PER on 14 Aug and MR/RPRF on 24 Sep. And its New Delhi Visa Office.

Applying for FBI PCC. My spouse is on H4 visa. The form has field for mentioning the last four digits of SSN. What should i write in her application ? :-X
