Mohon- Would you have any specific format for this affidevit? Or a notary will have it? Also, Is this Affidevit self attested? Do you think a covering letter should be attached to support for not being able to provide roles and duties on company's letter head? Any reply on this will be a great help.

You got to have something on company's letter head else it could most likely get rejected. If the employers don't provide anything, only affidevit won't work - i think so.

PCC is valid for one year. Start getting the PCC at the earliest.

Do i need to notarized all the documents again or can i submit those 2013 Notarized documents?

No you don't have to do it again. Its just a translation to English or French from some other language.

I have not done the PCC this year..my last PCC was issued on 31st March 2013..do i need to get latest PCC or the previous one will suffice?+

Yes you have to do it again as al PCC's are valid for one year. So the one which was issued last is not valid any more.

I only notarized few documents like my educational certificates & experience certificates which were duplicates...but those were written on English.

The applicaiton that I have sent last year was returned due to Cap limit. There was a UCI number that they wrote on my returned application. As I am planned to apply this year again, do I have to include last years UCI number with that ?

Just want to share about the PCC:

ur PCC submission validity is Three months. So when u want to submit ur PCC u should consider the date is not over three month from the issuing day.

Though PCC validity is One year but submission validity is Three months

Depends up to local Vo.

For New Delhi its usually 60 days and for recent trend they ask all of them in same package.

Yes brother u need to put that UCI number on Generic Application form...
