Congrats Zuziebaby! Yes ure meant to fill the forms n send alongside the intl passports. DHL fees shd differ depending on ur family size. But on the average, 15 to 16k shd be enuff to despatch it to Accra that is if ure in lagos.

If you like to save cost, follow UPS route.
DHL costs N15,190 in my time for sending passports 3wks ago. put your passport photographs too now and ensure you obey all CIC's instruction.

UPS does passport pick ups and passport deliveries.
For pick up from Accra its fixed nd its about N18,800 subject to rebate once your physical pacakge arrives... (weight dependent).
Sending to Accra on the otherhand too depending on the weight should not be more than N11500 for 1kg and less if its lesser.

UPS does passport pick ups and passport deliveries.
