Forms to be filled in CAPITAL or Small

Friends, may be silly but better to be clear ...

The forms need to be filled in CAPITAL or is it OK to fill in small alphabets ?

e.g. should it be NAME only or Name is OK ?

Not everything has to be mentioned in uppercase. Go with this format "Name".
