Forms for CAIPS

I want to apply for CAIPS. I have my sister in canada and i want her to apply for CAIPS for me. Please guide me what forms are to bee needed for this purpose.

do i fill and sign those forms OR my sister can fill and sign those forms for me.

Please give me the link of the required forms.
 
This topic on CAIPS is explained time and again on this forum. Please put the word CAIPS on the search area on top and plenty of articles are there including all the available links.

Your application was received by the Visa Office on 8th Feb 2010. As there is no communication after that ( you did not mention about any letter from CIC) from visa office, I hope, at any time you will be asked for Medical and Passport submission. Then why you need to go for CAIPS?. Do you think your application has been considerably delayed with the visa office?. Are you expecting an interview?. Did the CIC not answer for any of your enquiries?.

Once you complete all the formalities for CAIPS, it may take anothor month to receive CAIPS notes by your sister here in Canada. In the mean time you may perhaps already received and done the medical request. So think about it once again whether you need to go for CAIPS or not.

I didn't get any sort communication from the CHC Islamabad. Now My Visa office is CHC London like all the Pakistani applicants. I don't have any idea about the time line of CHC London. so i want to go for CAIPS. Maybe i could have some idea about my application.

You must be living in Canada or have someone in Canada to do the same for you. If you are living in Canada you can order your CAIPS notes or on behalf of someone else for free under the Privacy Act.

1) Go the site : http://www.cic.gc.ca/english/pdf/kits/forms/I... and fill in the form with the required details i.e.:

a] Visa post (e.g. Buffalo, NY etc.)
b] File number (e.g. BXXXX..)
c] Date of Birth
d] Your Postal address (In Canada address only)

2) Check on the box for the CAIPS notes request in the form.
3)Provide a photocopy of your valid permit/visa in Canada (Person requesting).
4) Write a cover letter requesting the notes (Person who requests writes that).

I'm wondering if any of you could answer me a few questions about the CAIPS forms:

Form 5475 E (the Access to Information and Personal Information Request):
"Section 3 - If you have already submitted your application"
Does the 'application' here refer to my PR application? How should I fill out the 'name of the office' and 'location of the office' if I submitted my application in Hong Kong?

Form 5563 B (the Authority to Release Personal Information form):
1. Requester's reference no.
Does it refer to the client ID number? (i.e. B123456789). If so, how is it different from the Visa file no.under 'REQUIRED INFORMATION' when I select 'CAIPS (information abroad)' under 'REQUEST FOR'?

2. ATIP file number
This is my first time filing the Access to Information and Personal Information Request form. In this case, what should I put down for the file number?

3. Under REQUEST FOR, I selected CAIPS (information abroad). If I submitted my application in Hong Kong, should I list the address of the Canadian Consulate in Hong Kong, or should I put down the address of the Canadian Embassy in Beijing?

we should send all of our CAIPS forms to Ottawa, not the regional offices? so we shouldn't send the forms to the following address?

for example,
