Format of the letter from Transport/Traffic office for Driving licence

Does anyone have a format for letter to be issued from Transport or Traffic police office for Driving licence ?

THERE IS NO SUCH FORMAT FOR THIS EVERY ISSUING AUTHORITY HAS ITS OWN FORMAT. IN 2010 I GOT ONE SLIP KINDDA THING SAYS MY NAME ENA DL no. ISSUING DATE.

There is no fixed format, but it should be on letter head of issuing authority. Check name spellings, DOB and d/L No also.
