Format of letter for 15 Hours of study per week

Dear All,

I had completed my Master's degree through distance education way back in 2004, I want to get letter from University that I had attended 15 hours of classed per week to complete this course.

Can any body please provide me the format of letter/wording which university need to write on letter. While sending application to university I want to send that sample wording to university to ensure that they have written in correct format.

No this is in India. I want to get a letter from university stating that I have attended 15 hours of classes per week (during my course Masters through distance education course) to ensure 25 points for my Masters degree.
