Form5707 or Form5645

I really need some assistance as I am confused (overwhelmed). Which family information do i need to fill up if i am applying from Philippines, form 5645 or 5707? Anyone can shed some light here please.

If you are applying online, it will specifically mention/display which form you need to submit. When I applied I was asked to fill up 5645.
