I just submitted form CTI 0001 to get a certificate of citizenship for my daughter, who was born outside of the US (I was born in Canada). It got rejected because "I didn't provide two identification documents for the child." however, I did submit a certified copy of her US passport and of her birth certificate. Is a birth certificate not a valid form of ID? what else can i use? I don't have a Social Security card (never got one though I have a notification of her SS number I could use). health insurance card with her name and my name on it?
