So, we've re-done this form twice now and might have to re-do a third time. In the personal history section is mentioning travel mandatory? I don't mind re-doing it as long as its accurate. What happens if you don't put down 1-2 vacations (reason i'm asking this question is that all this info is in an old passport that was never returned)?

I think it is better to put those vacations especially if there were done abroad. Put approximate dates if you don’t remember when you went exactly... and just write some letter to add that you did your best, but not sure if this is accurate since you don’t have old passport anymore.
P.S. maybe you have some photos from your vacations to check the date) sometimes this is how I check the date...
