For form 0008:

Is it ok to leave the "ext." part of telephone/alternate number/fax number blank - or do I also have to put N/A there still?

For form 5490:

- We really can't remember the exact month and date of our first contact, is it ok to leave it blank and just put the year?
- Do I still have to put N/A in numbers 20-23 if I previously answered that me and my spouse are already married?

I didn't include a fax number because we don't have one. You can't type "N/A" as that field only accepts numbers; I left that field blank...they'll understand...who honestly has a fax number these days?? E-mail is the way...not fax...old tech...

Write an explanation saying that you can't remember the exact month and day. Then just enter the year, but you should try harder to at least remember the month. Talk to family and friends, they may be able to help you remember. Worked for me.

If you are married, after Q19 it says "Go to question 24"...so no...just go to 24 and skip 20-23.

do not leave anything blank, if you dont remember the exact date, put star(**) and extension or fax number you can leave blank.

Try...I had to remember back to 2001...got the dates from a friend who remembered.

And the info posted by ashunandal is misleading. You don't need to do anything for 20-23 on IMM5491 if you are married...so you can leave those blank...not a problem...they will not expect you to enter anything...and it clearly states "go to question 24".

Fax number, if you don't have one, leave it blank...not a problem.

For dates, do not leave them blank...try your best to remember and write an explanation...

On family visas, they (still?) tend to be kind if you leave blanks. For other applications, they will reject applications and return them if you leave any blanks on the form (take a look at the skilled worker program threads, for example) asking you to complete the application and mail it back.

FWIW, my perspective is that it doesn't hurt to write in a bunch of n/a's once the form is printed as you go to sign them and if it avoids confusion so much the better

Somewhere in the guide it says to put * for dates you don't remember. IIRC it was for a specific date but try that for others and jot an explanatory note down.

Don't worry gurleegurl...leaving the fax number blank or questions 20-23 is not going to result in CIC rejecting your app and sending it back.

If on any of the forms it says to "skip ahead" or "go to questions ***", CIC understands that you will be leaving those sections blank...otherwise they wouldn't say to "skip ahead" or "go to questions ***"...right? Makes sense...you're fine...

As for the dates, if you don't remember, write that explanatory note and include it with the form...I couldn't remember the phone number when I lived in the US...I just explained that...no big deal.
