I forget to attached the photocopies of the passport as per the medical instruction with the medical form given to me by DMP

Although i have submitted original passport with the medical form

Do i need to send now with a request letter that i forget it in a hurry

its your DMP who shuld have done it, ask you DMP if they have done so or not.

Nothing to worry. your chc will make copies if they need them
these small mistakes are common among tensed Applicants.
