First Name and Surname

Hi Seniors,

I have only a First Name and the Surname. however, few of my documents have mentioned my name in the following interchanged orders .

• Bruce Wayne
• Wayne Bruce
• B. Wayne
• W. Bruce

do I have to get a same name affidavit for this? if so can someone please provide a sample of the same. your advice is highly valued.

You can prepare an affidavit for 'one and same person'.

I doubt if any affidavit is required. Name as per passport will anyways be considered for the application.. Others may correct me for sure

i agree with PK. You need the one & same person affidavit. We did the same for my wife.
