Filling out IMM08

Welcome, im filling out Generic Application Form for Canada [IMM 0008] and there is a place for UCI number, on medical examination i see UMI number but no UCI... Should i leave it blank?

If you don't have a UCI, then leave it blank. (Some people who have had dealings with CIC before may have UCI numbers, like those who have been IEC participants in the past, etc.)
