Fill up the work permit forms

Want to fill up the work permit application form myself.
So guys I need your help.

I am single...and feeling my form myself.

This application includes the following items:

1 Document Checklist [IMM 5488] (PDF, 30 KB)
2 Application for Work Permit Made Outside Canada [IMM 1295] (PDF, 347 KB)
3 Family Information [IMM 5645] (PDF, 59 KB)
4 Schedule 1 – Application for Temporary Resident Visa [IMM 5257 - Schedule 1] (PDF, 51 KB)
5 Statutory Declaration of Common-Law Union [IMM 5409] (PDF, 97 KB)
6 Use of a Representative [IMM 5476] (PDF, 55 KB)



Which form I have to fill and send to CIC?

Which form I have to fill and send to CIC?

1st, 2nd and 3 rd form you need to feel up . If someone going to feel you application you need to fell Use of a Representative [IMM 5476] as well. But representative should be authorized and legal .

@ Mannual; Thanks a lot for ur support.

With this forms which papers I have to submit?
I mean only the forms or (CV, Education certificate, passport photocopy, photograph, police clearance, medical certificate, offer letter from employee and LMO, job experience certificate etc) also have to attach?

Okay along with work permit application you need to submit your LMO job contract letter, passport copy ,photo, police report, enough bank balance, yours experience present and past. Education and other like marriage or birth certificate may require. And don't forget to pay work permit application fee. for madical after reviewing you application embassey will notice you when and where. thanks
