thanks again REYNOLD, you're really a Hero Member!
i was confused about this also when i applied...but just to make sure that i made no mistake, i included my middle name following my first names....this will be your consistent format already, when ever you write your name in the various forms that you need to fill in.

I have a follow-up question regarding this Given Name(s) thing. In the Schedule A (Background/Declaration) and Schedule 3 forms, under the Given Name(s) there are 2 blank boxes intended for it (first row and second row). Thus the second row is for the Middle Name? as the first row is for the First Name I suppose? This issue is an ongoing topic in another forum which I'm a member and there were conflicting answer about it.

tnx!

AFAIK, in Canada they dont recognize Middle names. but they are requiring us to write down our name as it appears in our passport...mine was simple because i only have a single name followed by my middle name, then surname, hence i wrote them down in each of the boxes, the second box i wrote my middle name....as i have said just follow the way your name is written in your passport... say in first box given names: John Carlo, middle name: Santos, surname 3rd box: Cruz

dear montu,
you really are so excited already...im glad for you...im planning to land first quarter next year...well regarding PR card, i have read in CIC site that you can have it filled already to lessen the hassle of filling it up during your landing...it saves your time and the attending officer's time as well.
