Extending stay as a visitor

My husband is applying to extend his stay as a visitor while his PR is in process. Just wondering if we make the payment online, do we need to submit IMM5401 (stamped receipt) or just submit the receipt from online? Is 100$ the correct payment?

Also, for proof of means of support - if I just write a letter as a guarantor stating I will financially support him, with bank statements, pay stubs and a letter from my employer describing my salary - will that be enough? Or do we need something more?

1) If you are applying online then you do not need to submit the payment receipt as you can only submit the online application once you pay the fee online through your mycic account.
2) your support letter with your bank statement and pay stubs will be good enough. If your husband has any bank account/cash etc then its worth mentioning that too.

Should the support letter be notarized??

Better make one affidavit of support letter that should be notarized.....
No need to attach receipt.
