I have doubt, about what to put in the section of Current Activity Occupation.
Actually Im fashion desginer from 09/2011 to NOW.. I mean what date I have to choose..
Do I have to say "from 09/2011 to 06/2012" for example?? or the "to" date I just leave it..?

Not good to leave blanks, so either write the month/year you sign the papers, or say "to present" or "till date" if the form will allow. I have a feeling it will only allow numbers, so 06/12 would be your best choice.

Yes, till the date you sign it.

Yes, that's exactly the feeling I had, but it appears to be the way to do it. I am sure they are used to seeing it like this.
