ECA Report - Name issue

in my ECA report the below line is mentioned

""The names on one or more of the academic credentials submitted for evaluation differ from the name indicated above."""

in my educational documents my surname not expanded but in my eca and Passport my surname was in expanded format?
1 will this be a problem?
2 do i need this point also in my LOE if yes then what can i mention ?

Yes, get an affidavit notarized and also a detailed LOE for this.

is it he one and same person affidavit do i need to get notarized for this purpose?
please confirm
and this means do i need to select the option yes for "do you know byany alias name " in my ee profile because right now i filled it as no. do i need to change this to Yes and then upload the affidavit?

No need to mention yes on alias question

simple affidavit for the name problem in educational documents.
