Doubt for filling form - Open work permit for spouse

Hello,

I would like to ask about the following:
I'm under a WP under NAFTA conditions. I would like to file an online application for an open WP for my wife.

My questions are:
1. In the form "Application to change coditions, extend my stay or remain in Canada", shall I fill it as me as the main applicant or her?
2. If me, should I thick the box above? (I'm applying for.
3. Can I do it online? if so, no info will have to be submitted, is that correct?

I just want to make sure and not making mistakes, as I just need an open work permit for her and not renewing or extending mine as is still valid until Jan 2011.

Thanks PMM, pretty clear now. One more question. I'm currently have a PR application processing in Buffalo. I have stated my wife as Temporary resident visitor, If she gets the Open Work permit, do I have to say something about it to our current PR application? an update or something about her change of status?
