Documents to CIO

Many are asking what documents should be sent to CIO. For sure, you need to send a IELTS and PCC.
I thought of starting this topic so someone who knows may reply to this thread and state exactly what is needed.
Maybe they can state whether it has to be original or not.

Or is it just that whatever documents we used to send to the visa office in the pre-June process (i.e., Appendix A) should now be sent to CIO.
