Documents required for LMO application for those who already working in Canada ?

Hi All,

I am currently working in Ontario as a full time employee under my one year PGWP which will expire in November. My employer is has agreed to file LMO for me to extend my work permit but has no idea how to proceed and what are documents required.

Please suggest me the procedure and if possible please provide a a document checklist form my side and employer .
