Documents name issue: one and the same person affidavit

I have same name in all document except company experience letter. What should I do for immigration to Canada?

Can't you get a corrected reference letter?
If no, then I guess affidavit is the only way to go and explain this anomaly in LOE as well.

Hi, format doesn't really matter, but I have heard that if you go to a Notary Public in India they already have formats. To me this format looks good enough https://www.edrafter.in/one-and-the-same-person/ (look at the image to the right)

I need one more help. I am preparing for PNP program for Canada. So at what stage do i need to produce this document? And from what level of authority should it be stamped?

Hi . Can you please confirm if we need just the affidavit i.e. one and same person format on stamp paper ordo we need to get it notarized too?
