Documents for eca on MCC

Hello, as i am applying for express entry and im going to need an ECA for my documents, and im totally new here, i wanted to know how to apply for physianapply, is it expensive and the documents should be sent to mcc by mail by me, or by the university. Im not clear at this point, they are send as scanned copies or mailed as physical copies to mcc?

The Medical Council of Canada accepts the electronic submissions of the following international (i.e., non-Canadian) medical credentials:

• Certified Identity Confirmation form (electronic signatures cannot be accepted)
• Identity document
• Medical degree or diploma
• Medical degree transcript
• Internship
• Postgraduate training
• Specialty certificate
• Medical licence or registration
• Name change document

Just a question on this, can we send only one document, or do we have to send all of these? i.e. is just sending the medical degree/diploma enough or should I be attaching also the transcript and the medical license and internship documents also?
