I overstayed roughly 2.5 months since my family had planned to take a vacation to visit me during that time anyway, and it was the easiest way for me to leave.

So I can answer No to the #6 question asking if I was ever denied or told to leave, and then just provide the dates in which I left on my timeline, and I should be okay? Example: [date left canada] Visiting visa extension submitted too late and denied extension, applicant leaves Canada.

when the notification about the extension came, did it specifically state, you had a certain number of days to leave? if so, then yes, you would have to say you were told to leave. if it didn't say anything, i don't think you need to say yes to that question. your call really.

The notification was not a departure order and specified only that the applicant (me) would be required to leave canada due to no status. There were no indication of amount of days.

So in this case, assuming I'm correct, I can answer no to that # 6 question, and just mention in my timeline the date in which I left and mention it was due to a late submitted visitor extension that was denied?

Yup I did! Here's what I did just to be sure:

At the same time, due to a sudden decline in health of her grandfather, sponsor and applicant agree that the best thing to do is to have applicant apply for Outland, that way she can take care of her grandparents and siblings while the paperwork is processed

Does that look fine?

looks good to me. just add the dates, and that way you can't be told you ommited information or "misrepresented" yourself. honestly, they don't scruitinize US files that much, so i can't see how you will run into any issues. good luck!
