We downloaded all the forms needed at the CIC site for outland spousal/common law sponsorship and there is only ONE document checklist shown under the section for Sponsor forms. Does both the sponsor and the applicant have to include a copy of this form in their application package? Or is there a separate document checklist for applicant?

If you mean the dovument checklist for PR application, yes just 1. Sponsor fills out forms marked for sponsor. Applicant (with sponsors help) fills out the rest.

Hmmmm....

I am sponsoring my wife under 'outland' spousal sponsorship.

As the sponsor, I used checklist IMM 5491 for me

For my wife (as Principal Applicant), I used the checklist included as 'Appendix A' - part of the region specific information guide, in this case, IMM3901 (the Western Europe one).

My sponsorship has been approved, now with London for PA assessment..... waiting.......

The Principal Applicant must locate their region on Part 3: Information Guides. Upon clicking on what part of the world the Principal Applicant resides they are presented with the Country Specific Instructions and their very own Document Checklist is located here.
