DO we need an INTERVIEW after DM for the Applicant?

I just want ask from everybody now, if the cic has told for some people that

"We received your application to sponsor XXXX XXXX on September 14, 2012.
We started processing your application on November 1, 2012.
We sent you a letter on November 21, 2012 about the decision on your application. Please consider delays in mail delivery before contacting us. "

and for some people

"We received your application for permanent residence on November 18, 2011.
We started processing your application on September 3, 2012.
Medical results have been received.
A decision has been made on your application. The office will contact you concerning this decision"

what is the different between these two?
"We sent you a letter on November 21, 2012 about the decision on your application. Please consider delays in mail delivery before contacting us."
means they have already made a decision & given the visa?
And,
"A decision has been made on your application. The office will contact you concerning this decision"
means they have not given the visa & need an interview?

And also I saw in one of the post that says,
Please note: At the time of your appointment, you and your family members, if any, will be required to present the original identification document(s) to the Canada Immigration Centre. A final decision concerning the granting of permanent residence status will be
made at that time. If permanent residence is granted, the name that will appear on the confirmation of permanent resident document is your name as it appears on your passport or identity documents."
what is this mean?

And also how long they take to issue the visa after see DM on ecas?
plzzzzzzzzzzzz I need everybody's comments who knows about these situations. if you have these experience with you, don't forget to share your (who has got their VISA for the permanent Resident) experience with us plzzzzz......... (I got an answer from one person . but i need to clear my mind more)

It's just saying when you arrive in Canada you will do the "landing" with immigration officer. They will ask you if any information has changed, such as you had a new baby but didn't include them on the application. As long as nothing has changed, you will get the PR.

if its DM, there will be no more interviews as what logic says.....
when its decision made, it means they have the verdict already on your application, approved or denied...........

In a different thread, I'm sure it was the Mexico one, someone knowledgeable answered this same question with both situations.
The first one is a refusal.
The second one is an approval.
That person wrote the difference is in the "medical results have been received" line.
Mine was the second one, did land.
Hope both are approvals, can someone else share?
