Do i need A POLICE RECORD

Hey everyone, hope i can get some help.
1) I want to know if i will need a police record when applying for Canadian visitors Visa?
2) Which forms do i need to fill out for the Visitors visa application?
I have filled out form IMM 5257, IMM 5645 and Schedule 1.. Let me know if a left out any forms... Thanks
