Do I have to send the original e-medical tracking sheet?

I'm just about ready to submit my application - I'm waiting for my daughter's documents to arrive from the UK to me here in the UAE.

I've just had a call from my Dad to say he forgot to put the info sheet in with the rest of the documents he's sent. He read it out ot me and it says words to the effect of "attach this form to your application..."

He's going to email me a copy of the form. Does anyone know if a copy is sufficient or do I have to send the original.
