Form 3901e states this

"You and all your family members who are not already Canadian citizens or permanent residents must fill out Appendix C and undergo and pass a medical examination, whether they will be joining you in Canada or not."

I have 2 sisters, a niece, a mum and dad and no dependants, none of them are considering moving to Canada.

Do they all HAVE to have medical exams incase at a later date i want to sponsor them?

If i don't include their medical exams will it affect my personal application?

For the purposes of a spousal/dependent sponsorship, your sisters, niece and parents are not family members, they are relatives. Family members in this case refers to the spouse and any dependent children of the spouse.
