Hello again everyone!

I sent my husband's PR application last week and am completely paranoid about his medical documents. I am convinced that the DMP that gave him his physical didn't give us the proper document to include with our application and on top of that I think his office mailed their copy to the wrong place. When we call they aren't any help as the receptionist doesn't seem to know what we are talking about and the Doctor is never around to talk to.

What does this elusive document look like? Is it the "green slip" that has been described in this forum? Our DMP's receptionist just gave us a photocopy of the forms and when we asked where it was sent she said Ottawa. Sigh.

So, in the likely event that my husband will have to do his physical again, can anyone suggest a good DMP in Toronto? We want to get another one ASAP and we don't want to end up in the same confusing situation as we did with this guy.

Ottawa seems right. I don't remember what the paper looked like that they gave me, but it was not a photocopy. As I understand it the meds sit in Ottawa until requested by the visa office processing your app.

Hmm.. so maybe there is hope for a green slip? If only the receptionist knew what we were talking about. The last thing she told us was that the only way to get one was to do the physical with the DMP again. Like we should pay again when we never got the proper document from them.. it doesn't make sense!

I'm sorry that you seem not to have gotten the right papers.

I thought Ottawa was off to, but as Alison said, journeyman also told me and it makes sense.

As to the green slip. It should include a photo of the applicant, and have the # 1017 in the upper right.
It is a pale green in areas surrounding where the information was written, the rest is white.

Anyhow, the "EFC" on the right in in that pale green and then 3 boxes (around the information on the left and the information on the right and hten also around the information at the bottom (just the last box that goes across the paper).

^I have mine in front of me, as I just had my medical this week.

So call or go back and tell them you need the correct form. Does it not say at the bottom of the form who gets the green, white, pink copies?

If they don't give it to you and tell you to do the medical again, maybe you could imply that you might file a complaint.

You should file a complaint anyway, but get the paper first! These people are sent cash paying patients by the government but they can't be bothered to do things in the right way? It isn't right.. If worse comes to worse, you may just have to get it done elsewhere. I used Dr. Kline in Toronto and they did the paperwork correctly.

What they gave me was about 10 pages of photocopies with all of the results of the physical.

My husband called and described the green form and the receptionist had no idea what he was talking about and suggested that he call the next day to speak with the doctor. When he called the next day the doctor wasn't there but had left him a message with the receptionist saying that he didn't have the green copy of the form (and it hadn't been given to my husband, that's for sure) and the only way to get it again was to get another physical but he was on vacation until August.

Yup. Frustrating.

So maybe I should call CIC and ask them what to do.

Go to another doctor and file a complaint with the CIC. You are NOT to get the results of the physical. You aren't to know anything about what it says unless you have a condition that needs treatment or further testing.

Really?!? I am so upset about this.

Will the fact that the DMP's receptionist gave us the results affect our application negatively?

I think we're just going to go for the complete do-over and get another physical and mail it in with a note stating why we had to go through the trouble and an official complaint about the DMP. We thought this DMP was going to be so great because he is also an MPP..

I"m sorry they weren't helpful and that he isn't there for another month. I too would file a complaint and sadly fork over the $ for another exam.

I made an appointment somewhere else. Better safe than sorry, I figure! I don't want to risk adding another month to our wait time. This way I can probably get it to the CPC in Mississauga before they have a chance to request it from me. Hopefully it still won't delay us :(
